If you are a Class Member, do not want to participate in this Settlement, and you want to keep the right to sue or continue to sue the Defendants or any of their Related Parties on your own for the Released Claims in this case, then you must take steps to get out of the Settlement Class. If your Exclusion Request is properly completed and timely received, you will receive no benefits or payment from the Settlement, but you will retain your right to sue on your own.
To be effective and to exclude you from this Settlement, an Exclusion Request must be completed and submitted, no later than October 7, 2026. All submitted Request for Exclusion Forms are subject to review and verification.
